Cardiac diseases are established risk factors for ischemic stroke. On the other hand, there is evidence from animal experiments and observational studies for a bidirectional relationship between cardiac diseases and ischemic stroke including increased cardiac dysfunction as well as cardiac events after ischemic stroke. In addition to an increased risk for vascular diseases, there are a number of further stroke associated associated sequelae influencing stroke patients’ prognosis, e.g. depression and cognitive decline.The SICFAIL cohort study aims to assess the prevalence of systolic dysfunction after ischemic stroke, to identify its determinants and to describe the natural course of systolic dysfunctions after ischemic stroke. To investigate the natural course of long term consequences follow-up examinations three and six months after stroke are performed for patients with the indication of possible disturbances of cardiac function at baseline.Furthermore, the long term risk of cardiovascular diseases up to five years after the initial ischemic stroke is to be determined and independent predictors for the observed risk patterns are to be identified.Moreover, the description of the natural course of stroke associated comorbidities such as depression and cognitive decline up to five years after the initial ischemic stroke, as well as the relationship of these comorbidities to cardiovascular disease are assessed.A total of 750 patients with ischemic stroke are recruited in the stroke unit of the University Hospital of Würzburg with follow-ups up to 5-year after the event. All patients with evidence for cardiac dysfunction in the acute phase are examined in the common joint epidemiologic survey unit premises of the ICE-B and the Comprehensive Heart Failure Center three and six months after stroke. All patients are contacted by phone or post annually after the initial stroke and interviewed about their state of health.The first results of the SICFAIL study were presented at the congress of the European Stroke Organisation 2016 inBarcelona.

The study is a cooperation project between the Institute for Clinical Epidemiology and Biometry (ICE-B) of the University of Würzburg and the Department of  Neurology at the University Hospital of Würzburg within the Comprehensive Heart Failure Center.
